We recommend you save these forms (see below) to your computer and then open them. You may print the above forms and mail or e-mail them back to us. The Adobe files are much smaller and therefore faster to download. The forms are available in two different formats:
If you get a Virus Message when downloading one of the above files. . . If you get a warning that a virus is attached to one of the above files, we assure you there is no virus. What this usually means is that you have your browser set to high security. However, if you are still uncertain, scan it with your virus protection program before opening it, or call your internet provider for assistance. Saving These Files to Your Computer By clicking on the form name from within your browser (Windows Internet Explorer) or e-mail program (Outlook, Modzila, Eudora, etc.) usually a window similar to the one below will appear asking whether you wish to open or save the file - you are usually given a choice. unless you have un-clicked the box, "Always ask before opening this type of file" previously.
When you click on "Open it", and then "OK" your word processor should automatically open. Next, save the form from within your word processor (File > Save As). You may rename it at that time if you wish. You may then enter information and send it back to us via e-mail. To do this click "File" > "Send To" > "E-mail As Attachment", and your e-mail program should automatically open with the file already attached to the e-mail message. If your word processor does not open, click on "Save it to Disk". Save it to a directory on your hard drive where you save your data files (i.e. My Documents). Then open your word processor, go to the folder (directory) where you saved the form and open it. You may print and mail the completed form to us, or type in your information and e-mail it back to us. Remember to save the completed form. Adobe Files You must have Adobe Acrobat Reader installed on your computer to read and print the PDF files. If you do not have Adobe Acrobat Reader installed on your computer, click here to obtain a free copy from Adobe. Once you have Adobe Acrobat Reader installed on your computer, the PDF files will load automatically into Adobe. You can only print them off with Acrobat Reader. If you have a full version of Adobe Acrobat, you may type in your information and e-mail the form to us. |
